I, ________________________________________ HEREBY ASSUME ALL THE OF THE RISKS OF PARTICIPATING IN ANY AND ALL ACTIVITIES OF Lion’s Heart Family Martial Arts, including but not limited to any risks associated with the use of equipment, any risks associated with partner contact, any risks associated with training in martial arts under Lion’s Heart Family Martial Arts (or any of their persons or entities). 

I certify that I am physically capable of the basic demands of this activity and have not been advised not to participate by a qualified medical professional. I certify that there are no health-related problems that prevent me from participating in any activity through Lion’s Heart Family Martial Arts (or any of their persons or entities).

I acknowledge that this accident waiver and release of liability form will be kept and used by Lion’s Heart Family Martial Arts (or any of their persons or entities) and that it will govern my actions and responsibilities while training here. 

By signing this accident waiver and release of liability form, I hereby take action for myself (or for the person I am signing for if they are under eighteen years of age) and recognize the following:

1. I WAIVE, RELEASE, AND DISCHARGE Lion’s Heart Family Martial Arts (or any of their persons or entities) from any and all liability, including but not limited to any risks associated with the use or equipment, any risks associated with partner contact, any risks associated with training.
2. I HOLD HARMLESS AND PROMISE NOT TO SUE Lion’s Heart Family Martial Arts (or any of their persons or entities) from any and all risks associated with martial arts training under Lion’s Heart Family Martial Arts.

I acknowledge that this activity may test a person’s physical and mental limits and carries with it the risk of minor to serious injury and/or illness.

I understand that while participating in any activity related to Lion’s Heart Family Martial Arts, I may be photographed. I permit any photos or videos of myself to be used by Lion’s Heart Family Martial Arts for any legitimate purpose.

The accident waiver and release of liability form shall be constructed broadly to provide a release and waiver to the maximum extent available.

I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.
